What is biological age, and can a blood test measure it?
Biological age is an attempt to express how old your body appears to be functioning, as opposed to how many years you have lived. It is an estimate, not a measurement: different methods produce different answers for the same person on the same day, and no single test establishes it reliably. What is biological age trying to describe?
The idea behind biological age is simple and worth taking seriously. Two people born on the same day are not in the same biological condition. One recovers quickly from a hard week, sleeps well, and holds a stable metabolic profile. The other does not. Chronological age cannot tell them apart. Something else clearly can.
Biological age is the attempt to put a single number on that difference, by expressing a person's biological condition as the chronological age it most resembles. The intent is legitimate. The problem is what happens when the attempt is packaged as a result.
What do independent sources say about biological age tests?
They say roughly what this page says, which is worth knowing before you spend anything.
Cleveland Clinic Health Essentials, in "Biological Age: What It Is and How You Can Measure It" (consulted 29 August 2026), describes the two families of method in use, epigenetic clocks based on DNA methylation and clinical algorithms combining routine measurements, and then adds the caveat that matters about the consumer tests sold online: "It's not clear how reliable these tests are."
The BioTwin blog already points to two peer-reviewed papers on the same question, one in Aging Cell on metabolomic age and chronic disease risk, one in Advanced Science on a metabolomics-based biological age predictor associated with mortality.
The honest limit applies to all three. These are population studies and population-level commentary. An association measured across a cohort is not a promise about one person, and none of these sources evaluates BioTwin.
How do people try to measure biological age?
There is no single method, and that is the first thing to understand.
Blood chemistry models take a set of routine markers and combine them through a statistical model into an age-like figure. Epigenetic clocks read chemical marks on DNA, usually methylation, and estimate an age from those patterns. Functional tests measure grip strength, gait speed, lung function or cognitive performance and place a person relative to age norms. Telomere length has been used, and has fallen out of favour as a standalone estimate.
These methods do not agree with each other. They were trained on different populations, they optimise for different outcomes, and they answer slightly different questions. A person can be told they are five years younger by one method and three years older by another, in the same week, without either method being fraudulent. It simply means the number is model-dependent.
Can a blood test really measure your biological age?
Not in the way the phrase suggests. A blood test can feed a model that produces an estimate. It cannot measure an age, because an age of that kind is not a physical quantity present in the sample.
This distinction is not a technicality. It changes what you should do with the answer. A measurement can be checked. An estimate has to be interpreted, and it is only as good as the model that produced it and the population that model was built on. Anyone selling you a biological age owes you that sentence, and most do not say it.
Why does a single biological age number mislead?
Because it is presented as a verdict, and a verdict invites the wrong response.
You are 36 but biologically 32. You are 50 but biologically 44. You gained two years. You lost three. The number is simple, clickable, and easy to market. It is also incomplete, and the incompleteness runs in three directions.
It hides which part of you is doing what. A blended figure mixes metabolic, oxidative, immune, muscle-related, recovery-related and nutrition-linked signals into one output. Those axes do not move together, and they do not respond to the same actions. A decomposed picture is more useful than one blended number, even when the blended number is more satisfying to read.
It hides movement. A body is not a static number. A person's biological state moves with sleep, nutrition, stress, travel, alcohol, exercise, illness, fatigue and recovery. A score taken once may be interesting. A score tracked over time is more useful.
It does not tell you what to do. This is the practical failure. Knowing you are biologically 44 rather than 50 does not name a single action. It ends the conversation instead of starting one.
What is a trajectory, and why is it the better question?
A trajectory is direction, pace and resilience: which way your biology is moving, how fast, and how quickly it returns to your own normal after something disturbs it.
It is a better question for a plain reason. Direction is actionable and a verdict is not. If you know your trajectory, you can ask what improves it and what destabilises it, and then test the answer on yourself. If you only have a number, you can compare yourself to strangers.
The questions worth asking are these:

- What improves my trajectory?
- What destabilises it?
- How fast do I recover from stress?
- What does my best state look like?
- Can I recreate the conditions that produced it?
None of those questions has an answer inside a single measurement. All of them have an answer inside a series.
Why does your own baseline matter more than the population average?
Because a population range tells you whether you resemble other people, and your own history tells you whether you are changing.
Standard reference ranges are built to flag people who fall outside a normal distribution. That is a reasonable design for a screening tool and a poor one for someone who is trying to improve. Most people who want to follow their health sit comfortably inside every reference range and still change considerably over a year. Population norms cannot see that. Your own previous sample can.

How often would you have to measure to see a trajectory?
More than once, and that is the whole point.
One sample is a baseline. It is readable on its own and it is where everyone starts. Two samples give you a direction. Several across a year give you a pace, and let you see whether something you changed actually moved your biology rather than only your mood. Results appear in your portal within 1 to 3 months of your sample arriving, and from the second kit onward you also see how each score has moved since the previous one.
